• Dependent Identity Statement of Educational Purpose Verification Worksheet
This form must be printed and completed and signed by hand either in person at the Hollins University Office of Scholarships and Financial Assistance or in person with an authorized Notary Public. An unexpired government-issued photo identification must be presented at the time you complete the form. If completing the form in person with a Notary Public, you must either drop the form by the Hollins University Office of Scholarships and Financial Assistance or mail it to Hollins University, Office of Scholarships and Financial Assistance, 7916 Williamson Road, Box 9718, Roanoke, VA 24020. If mailing, make sure not to forget to include your completed notarized worksheet and the copy of your unexpired government-issued photo identification that was presented to and notarized by the Notary Public.

• IRS Tax Return Transcript Request Process
This form gives instructions on how to request a free IRS Tax Return Transcript if needed to complete the verification process.
• Refund Request Form
This form is to be completed by any student who has a credit on their student account and wishes to receive a refund for those funds to use for college or living expenses. Refunds are issued once a week and usually take 7 to 10 business days to process. Please submit your Refund Request Form to the Office of Scholarships and Financial Assistance on Fridays by 2:30 pm Please note that refunds for fall/spring terms won’t be processed until after the term has begun and your financial aid has disbursed.

• Title IV Authorization Form
This is an optional form. By completing this form, students give Hollins University authorization to use excess federal funds/aid on their account toward prior year charges, current charges, or future charges.
• The Family Educational Rights and Privacy Act (FERPA) Form
This is an optional form. Students may use this form to give the Office of Scholarships and Financial Assistance permission to share information about his/her financial aid with a parent, spouse, or guardian.
• Graduate Financial Aid Application
Graduate students should complete this form to let our office know their enrollment plans for the upcoming school year. • MALS Scholarship Application
This is a graduate scholarship application for MALS students.
• Virginia Tuition Assistance Grant Application (VTAG)
This form must be completed and signed by hand and either mailed to us at Hollins University, Office of Scholarships and Financial Assistance, 7916 Williamson Road, Box 9718, Roanoke, VA 24020 or dropped by the Office of Scholarships and Financial Assistance before July 31st. Virginians who are full-time may be eligible.
• Horizon Student Scholarship Assistance Application
Adult Horizon Students who wish to apply for the Betty Trinkle Freeman, Lucy, Hazelgrove and/or the William Randolph Hearst Scholarships will want to complete this form.
• Virginia Voter Registration Form (elections.virginia.gov)
One way to ensure that Federal and State financial aid continues to be a viable program is to vote for your local/state/federal representatives. You can print this form and mail it to the address listed. Virginia residents can also use their Voter’s Registration Card as a supporting document for TAG.
• SSN Card
When you completed your FAFSA, a match was completed with the Social Security Administration. According to the SSA, your Social Security number did not match the database. You have 2 options: 1) If you believe your SSN is valid, send a copy of your SSN card to the Office of Scholarships and Financial Assistance at Hollins. You can fax it to 540-362-6093. 2) If you believe it is incorrect, please contact the Social Security Administration.
• Virginia Driver’s License
If requested, we may need a copy of your Virginia Driver’s License as documentation to support your residency in the state of Virginia for the TAG grant.
• Certification Signed by Student
In order for us to continue processing your financial aid and secure your scholarship funding, you will need to log on to the Hollins Information System and accept or decline your award by navigating to the Main Menu>Student Services>Financial Aid>Award Offer>Accept/Decline Aid>Hit “submit”.
• Entrance Counseling for Student Loans (studentaid.gov)
This is an online form that must be completed by first-time federal loan borrowers in order for the loan to disburse. This form ensures the student understands the terms and conditions of the loan and their rights and responsibilities as a loan borrower.
